Medicare fee-for-service inpatient stays only. Many hip and knee replacements are now done as outpatient cases, and critical access, VA and military hospitals are not in this file at all, so total case volume is higher. CMS publishes a hospital's number for a procedure only when it had 11 or more Medicare discharges, so these counts are a floor.

Denominators, definitions and sources

  1. Hospitals come from CMS's Hospital General Information file, updated Jul 22, 2026. Each one is placed at the center of its ZIP code area from the Census Bureau's 2024 files. Where the ZIP is a post office box, it is placed at the center of its city. Distances are straight-line, not driving, miles from downtown Nashville.
  2. Procedure groups are Medicare inpatient discharges for 2024 by hospital and diagnosis related group. Joint covers hip and knee replacement and revision, bilateral joint procedures, shoulder and other upper limb joint replacement, and hip replacement after a fracture. Open heart covers valve and other major cardiothoracic surgery and bypass surgery. Cranial covers craniotomy and the endovascular brain procedures CMS groups with it.
  3. Surgery centers come from CMS's surgery center quality file (2024 release). A center is counted as doing orthopedic cases when CMS published its measure on hospital visits after orthopedic procedures, which CMS builds from Medicare claims for patients 65 and over. CMS holds back that result for centers with too few such cases, and 26 more centers in the radius fall in that group, so 39 is a floor.
  4. Each hospital is one CMS provider number, and a few of them cover more than one campus. One hospital listed twice at the same address is counted once. Hospitals near the 150 mile line were placed by street address.
  5. Discharge figures of 1,000 or more are rounded on purpose.
